    Evolução audiométrica em usuários de implante coclear multicanal

    PURPOSE: to evaluate how long after activation one can achieve the desired auditory thresholds in individuals multichannel cochlear implant users from different age groups. METHOD: a retrospective cross-sectional from a database, approved by CEP / FMUSP, under number 779/06. Individuals implanted between January 2005 and September 2008, whose files had preoperative audiometric data and 3, 6 and 12 months after CI were included. Seventy-two patients fulfilled the criteria, and they were divided in two groups according to age: group I (17 year old) and GII (18-66 year old). Analysis of variance (ANOVA) of one factor in order to verify the interaction between the mean threshold and time after CI were used. RESULTS: we observed that 15 of the GI have reached around 30dBNA thresholds at 3 months after implant. The same happened to 26 patients of GII. We found significant interaction between time (F (3.140) = 91.973, p = 0.00) and the average hearing thresholds. Post-hoc analysis in the GI group were statistically significant differences for comparisons between HA and 3 months after implantation (p = 0.00) and between 3 and 12 months (p = 0.28). For GII only for comparison between HA and 3 months (p = 0.00). CONCLUSION: a significant hearing change may be observed beginning from three months after IC, but this result is not found in all implanted subjects.

    Estudo da função auditiva em indivíduos com HIV/AIDS submetidos e não submetidos à terapia antirretroviral

    PURPOSE: To characterize the audiologic manifestations in individuals with HIV/AIDS (Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome) submitted and not submitted to antiretroviral therapy. METHODS: The audiologic evaluation was carried out in 28 individuals in Research Group I (RGI) with HIV/AIDS submitted to antiretroviral therapy; 24 individuals in Research Group II (RGII) with HIV/AIDS not submitted to antiretroviral therapy, and 45 healthy individuals in the Control Group (CG). The audiologic tests that comprised this study were: Conventional Pure Tone Audiometry, Speech Audiometry and High Frequency Audiometry. RESULTS: There were differences between groups RGI and RGII regarding conventional audiologic evaluation and high frequency audiometry. There were higher thresholds for both Conventional and High Frequency Audiometries when compared with CG subjects, however without significant difference among RGI and RGII. CONCLUSION: Individuals with HIV/AIDS present more abnormal results in Conventional Pure Tone Audiometry and High Frequency Audiometry than healthy individuals (CG), which indicates impairment in the hearing system. Individuals with HIV/AIDS present more abnormal findings in High Frequency Audiometry when compared to Conventional Pure Tone Audiometry. There was no difference between individuals with HIV/AIDS submitted (RGI) and not submitted (RGII) to antiretroviral therapy regarding Conventional Pure Tone Audiometry and High Frequency Audiometry.

    Auditory brainstem response in neonates: influence of gender and weight/gestational age ratio

    OBJECTIVE: To investigate the influence of gender and weight/gestational age ratio on the Auditory Brainstem Response (ABR) in preterm (PT) and term (T) newborns. METHODS: 176 newborns were evaluated by ABR; 88 were preterm infants - 44 females (22 small and 22 appropriate for gestational age) and 44 males (22 small and 22 appropriate for gestational age). The preterm infants were compared to 88 term infants - 44 females (22 small and 22 appropriate for gestational age) and 44 males (22 small and 22 appropriate for gestational age). All newborns had bilateral presence of transient otoacoustic emissions and type A tympanometry. RESULTS: No interaural differences were found. ABR response did not differentiate newborns regarding weight/gestational age in males and females. Term newborn females showed statistically shorter absolute latencies (except on wave I) than males. This finding did not occur in preterm infants, who had longer latencies than term newborns, regardless of gender. CONCLUSIONS: Gender and gestational age influence term infants' ABR, with lower responses in females. The weight/gestational age ratio did not influence ABR response in either groups.

    Long latency auditory evoked potentials in children with cochlear implants: systematic review

    The aim of this study was to analyze the findings on Cortical Auditory Evoked Potentials in children with cochlear implant through a systematic literature review. After formulation of research question and search of studies in four data bases with the following descriptors: electrophysiology (eletrofisiologia), cochlear implantation (implante coclear), child (criança), neuronal plasticity (plasticidade neuronal) and audiology (audiologia), were selected articles (original and complete) published between 2002 and 2013 in Brazilian Portuguese or English. A total of 208 studies were found; however, only 13 contemplated the established criteria and were further analyzed; was made data extraction for analysis of methodology and content of the studies. The results described suggest rapid changes in P1 component of Cortical Auditory Evoked Potentials in children with cochlear implants. Although there are few studies on the theme, cochlear implant has been shown to produce effective changes in central auditory path ways especially in children implanted before 3 years and 6 months of age.

    Caracterização eletrofisiológica da audição em prematuros nascidos pequenos para a idade gestacional

    PURPOSE: To characterize the Auditory Brainstem Response (ABR) of small for gestational age preterm newborns and to compare the findings to those of appropriate for gestational age premature newborns in order to verify whether the small for gestational age condition is a risk factor for hearing loss. METHODS: This prospective cross-sectional multicenter study evaluated 72 preterm newborns of both genders (35 small and 37 appropriate for gestational age), who were born at 30 to 36 weeks of gestational age and were evaluated before hospital discharge. Only newborns with present transient evoked otoacoustic emissions and tympanometry type A were included. The ABR was performed with click stimuli. The quantitative data analysis was performed using mean and standard deviation measures for each group. For qualitative analysis, the ABR results were classified as normal or altered according to the absolute latencies of waves I, III, V and interpeaks I-III, III-V, I-V. The analysis was carried out considering the age of the newborn at the time of examination. RESULTS: Alterations were evident in 32 newborns (44.44%), being 15 small (43%) and 17 appropriate for gestational age (46%), with no between-groups difference. Of the 15 small for gestational age newborns with altered ABR, six presented as auditory risk only the small for gestational age condition. In the group of adequate for gestational age newborns, there was a higher occurrence of alteration in males. CONCLUSION: There was no difference in responses of auditory evoked potential between small and appropriate for gestational age preterm newborns. Therefore, the condition does not behave as a risk factor for retrocochlear impairment.

    Hearing complaints and the audiological profile of the users of an academic health center in the western region of São Paulo

    INTRODUCTION: Few population-based studies have quantified hearing levels in Brazil; additional studies on this subject are needed. PURPOSE: The purpose of this study was to characterize hearing complaints and the audiological profile of the population served by the Clinical Audiology Service of an Academic Health Center in the western region of São Paulo, Brazil, between 2003 and 2008. An additional aim was to check whether there is a positive association between the signs/symptoms and type of hearing loss. METHODS: This was a retrospective study of the records of 2,145 patients. The health history, tonal and vocal audiometry, and imitanciometry findings were analyzed. RESULTS: The mean age of the patients was 20.6 years. The majority of the subjects had normal hearing thresholds, and the prevalence of hearing loss was approximately 35%. As the patient's age increased, the frequency of conductive hearing loss decreased and that of sensorineural hearing loss increased. There was a tendency toward hearing loss worsening with age. CONCLUSION: Hearing complaints can predict the type of hearing loss; therefore, they should always be valued because they can be used as a form of screening and thus help to determine the diagnostic hypothesis. This could help to reduce the gap between the patient's perception of the complaints and the audiological assessment and thus improve the prognosis.

    Long latency auditory evokedpotentials in malnourished children

    PURPOSE: To characterize the findings obtained in the long latency auditory evoked potentials in malnourished children, as well as to compare them to the results obtained for children at the same age who present typical development. METHODS: Sixty-five children aged between 7 and 12 years old, of both genders, were evaluated. The control group consisted of 34 children with normal development and the study group consisted of 31 children diagnosed with malnutrition. All children underwent conventional pure tone audiometry, acoustic impedance measurement, dichotic digit test and long latency auditory evoked potentials. RESULTS: The study group presented a statistically significant difference for latencies of the components P1, N1 and P300. The latencies of these components were higher than those of the control group. Concerning the types of alterations found in the study group, increased latency for P1 and P300 and the absence of response in N1 were predominant. The control group showed a statistically significant difference in the amplitude of P300 when compared to the right and left ears. CONCLUSION: Malnourished children present with changes in the long latency auditory evoked potentials, suggesting a deficit in central auditory pathways and alterations in the processing of acoustic information.
