    Perspectives on invasive and noninvasive ventilatory support for critically ill patients are evolving, as much evidence indicates that ventilation may have positive effects on patient survival and the quality of the care provided in intensive care units in Brazil. For those reasons, the Brazilian Association of Intensive Care Medicine (Associacao de Medicina Intensiva Brasileira - AMIB) and the Brazilian Thoracic Society (Sociedade Brasileira de Pneumologia e Tisiologia - SBPT), represented by the Mechanical Ventilation Committee and the Commission of Intensive Therapy, respectively, decided to review the literature and draft recommendations for mechanical ventilation with the goal of creating a document for bedside guidance as to the best practices on mechanical ventilation available to their members. The document was based on the available evidence regarding 29 subtopics selected as the most relevant for the subject of interest. The project was developed in several stages, during which the selected topics were distributed among experts recommended by both societies with recent publications on the subject of interest and/or significant teaching and research activity in the field of mechanical ventilation in Brazil. The experts were divided into pairs that were charged with performing a thorough review of the international literature on each topic. All the experts met at the Forum on Mechanical Ventilation, which was held at the headquarters of AMIB in Sao Paulo on August 3 and 4, 2013, to collaboratively draft the final text corresponding to each sub-topic, which was presented to, appraised, discussed and approved in a plenary session that included all 58 participants and aimed to create the final document.

    Implementation of Tele-ICU during the COVID-19 pandemic

    Objective: To describe the implementation of a Tele-ICU program during the COVID-19 pandemic, as well as to describe and analyze the results of the first four months of operation of the program. Methods: This was a descriptive observational study of the implementation of a Tele-ICU program, followed by a retrospective analysis of clinical data of patients with COVID-19 admitted to ICUs between April and July of 2020. Results: The Tele-ICU program was implemented over a four-week period and proved to be feasible during the pandemic. Participants were trained remotely, and the program had an evidence-based design, the objective being to standardize care for patients with COVID-19. More than 100,000 views were recorded on the free online platforms and the mobile application. During the study period, the cases of 326 patients with COVID-19 were evaluated through the program. The median age was 60 years (IQR, 49-68 years). There was a predominance of males (56%). There was also a high prevalence of hypertension (49.1%) and diabetes mellitus (38.4%). At ICU admission, 83.7% of patients were on invasive mechanical ventilation, with a median PaO2/FiO(2) ratio < 150. It was possible to use lung-protective ventilation in 75% of the patients. Overall, in-hospital mortality was 68%, and ICU mortality was 65%. Conclusions: Our Tele-ICU program provided multidisciplinary training to health care professionals and clinical follow-up for hundreds of critically ill patients. This public health care network initiative was unprecedented and proved to be feasible during the COVID-19 pandemic, encouraging the creation of similar projects that combine evidence-based practices, training, and Tele-ICU.

    Impact of a respiratory ICU rotation on resident knowledge and confidence in managing mechanical ventilation

    Objective: To develop and apply a competency-based test to assess learning among internal medicine residents during a respiratory ICU rotation at a university hospital. Methods: We developed a test comprising 19 multiple-choice questions regarding knowledge of mechanical ventilation (MV) and 4 self-assessment questions regarding the degree of confidence in the management of MV. The test was applied on the first and last day of a 30-day respiratory ICU rotation (pre-rotation and post-rotation, respectively). During the rotation, the residents had lectures, underwent simulator training, and shadowed physicians on daily bedside rounds focused on teaching MV management. Results: Fifty residents completed the test at both time points. The mean score increased from 6.9 +/- 1.2 (pre-rotation) to 8.6 +/- 0.8 (post-rotation; p < 0.001). On questions regarding the approach to hypoxemia, the recognition of patient-ventilator asynchrony, and the recognition of risk factors for extubation failure, the post-rotation scores were significantly higher than the pre-rotation scores. Confidence in airway management increased from 6% before the rotation to 22% after the rotation (p = 0.02), whereas confidence in making the initial MV settings increased from 31% to 96% (p < 0.001) and confidence in adjusting the ventilator modes increased from 23% to 77% (p < 0.001). Conclusions: We developed a competency-based test to assess knowledge of MV among residents before and after an rotation in a respiratory ICU. Resident performance increased significantly after the rotation, as did their confidence in caring for patients on MV.

    Diaphragmatic ultrasound: a review of its methodological aspects and clinical uses

    ABSTRACT The diaphragm is the main muscle of respiration, acting continuously and uninterruptedly to sustain the task of breathing. Diaphragmatic dysfunction can occur secondary to numerous pathological conditions and is usually underdiagnosed in clinical practice because of its nonspecific presentation. Although several techniques have been used in evaluating diaphragmatic function, the diagnosis of diaphragmatic dysfunction is still problematic. Diaphragmatic ultrasound has gained importance because of its many advantages, including the fact that it is noninvasive, does not expose patients to radiation, is widely available, provides immediate results, is highly accurate, and is repeatable at the bedside. Various authors have described ultrasound techniques to assess diaphragmatic excursion and diaphragm thickening in the zone of apposition. Recent studies have proposed standardization of the methods. This article reviews the usefulness of ultrasound for the evaluation of diaphragmatic function, addressing the details of the technique, the main findings, and the clinical applications.

    Identifying decreased diaphragmatic mobility and diaphragm thickening in interstitial lung disease: the utility of ultrasound imaging

    Objetivo: Investigar a aplicabilidade da ultrassonografia do diafragma na doença pulmonar intersticial (DPI). Métodos: Por meio da ultrassonografia, pacientes com DPI e voluntários saudáveis (controles) foram comparados quanto à mobilidade diafragmática durante a respiração profunda e a respiração tranquila, à espessura diafragmática no nível da capacidade residual funcional (CRF) e da capacidade pulmonar total (CPT) e à fração de espessamento (FE, espessamento diafragmático proporcional da CRF até a CPT). Foram também avaliadas correlações entre disfunção diafragmática e variáveis de função pulmonar. Resultados: Entre os pacientes com DPI (n = 40) e os controles (n = 16), a média da mobilidade diafragmática foi comparável durante a respiração tranquila, embora tenha sido significativamente menor nos pacientes durante a respiração profunda (4,5 ± 1,7 cm vs. 7,6 ± 1,4 cm; p < 0,01). Os pacientes apresentaram maior espessura diafragmática na CRF (p = 0,05), embora tenham também apresentado, devido à menor espessura diafragmática na CPT, menor FE (p < 0,01). A CVF em porcentagem do previsto (CVF%) correlacionou-se com a mobilidade diafragmática (r = 0,73; p < 0,01), e um valor de corte < 60% da CVF% apresentou alta sensibilidade (92%) e especificidade (81%) na identificação de mobilidade diafragmática reduzida. Conclusões: Com a ultrassonografia, foi possível demonstrar que a mobilidade diafragmática e a FE estavam mais reduzidas nos pacientes com DPI do que nos controles saudáveis, apesar da maior espessura diafragmática na CRF nos pacientes. A mobilidade diafragmática correlacionou-se com a gravidade funcional da DPI, e um valor de corte < 60% da CVF% mostrou ser altamente acurado na identificação da disfunção diafragmática por ultrassonografia.
