    Analysis of the PvuII and XbaI polymorphisms in the estrogen receptor alpha gene in girls with central precocious puberty: a pilot study

    Background: Precocious puberty (PP) is defined as premature pubertal development. Its consequences surpass the physical evidence of sexual maturity with the premature epiphyseal closure of the long bones and the reduction of adult stature by varied degrees. Central PP is characteristically dependent on GnRH and most of its causes are not completely known. Altered estrogen action is also believed to be involved in the genesis of PP. In fact, estrogen receptor alpha (Rea) gene polymorphisms may be associated with early age at menarche. The objective of this study was to investigate the relationship between Rea gene polymorphisms (PvuII and XbaI) and the occurrence of central PP. Methods: A total of 73 girls with central PP and 101 girls with normal pubertal maturation were evaluated. Both groups were genotyped for the PvuII (T/C) and XbaI (A/G) polymorphisms in the Rea gene. Results: The frequency distribution of the XbaI (p = 0.28) and of the PvuII (p = 0.12) genotypes, as well as the XbaI and PvuII allelic variants (p = 0.23 and p = 0.86, respectively), did not differ between the groups. Conclusion: The PvuII and XbaI Rea gene polymorphisms do not appear to be related to development of central PP.

    Impact of functional training on geometric indices and fractal correlation property of heart rate variability in postmenopausal women

    BackgroundTo evaluate the influence of functional training on the geometric indices of heart rate variability (HRV) and fractal correlation properties of the dynamics of heart rate in menopausal women. MethodsOf 39 women who were in the period of menopause for more than a year and who did not practice any regular physical activity were divided into: Functional training group (FTG=504.5years; 67.64 +/- 11.64kg; 1.5 +/- 0.05m) that executed the functional training (FT) and all proposals by reviews and the Control group (58.45 +/- 4.8years; 66.91 +/- 13.24kg; 1.55 +/- 0.05m) who performed all assessments but not FT. The training consisted of 18weeks (three times a week) and the volunteers performed three sets of 11 functional exercises followed by a walk in each of the sessions. The autonomic nervous system modulation was evaluated by analysis of HRV and the indices obtained were: RR intervals, RRTRI, TINN, SD1, SD2, SD1/SD2, qualitative analysis of Poincare plot and DFA (alfa-1, alfa-2 and alfa-1/alfa-2). The Student's t-test for unpaired samples (normal data) or Mann-Whitney test nonnormal data) were used to compare the differences obtained between the final moment and the initial moment of the studied groups (p<.05). ConclusionWere observed in the FTG: increased SD1 (CG 0.13 +/- 4.00 vs. 3.60 +/- 8.43), beat-to-beat global dispersion much greater as an increased in the dispersion of long-term RR intervals and increased fractal properties of short-term (1) (CG -0.04 +/- 0.13 vs. 0.07 +/- 0.21). FT promoted a beneficial impact on cardiac autonomic modulation, characterized by increased parasympathetic activity and short-term fractal properties of the dynamics of the heart rate.

    Home-based exercise therapy for treating non-specific chronic low back pain

    OBJECTIVE: To evaluate pain, functional capacity, and quality of life of patients with non-specific chronic low back pain, after home-based exercise therapy with different kinds of supervision. METHOD: Thirty individuals of both gender, between 18 and 65 years old, performed the proposed exercises three times a week, for eight weeks. Group A (N = 17) performed the exercises after a single supervised session. Group B (N = 13) was supervised once a week at the rehabilitation center. Both groups received a booklet with instructions, and questionnaires to evaluate pain, functional capacity and quality of life; during the initial evaluation, after four and eight weeks. RESULTS: There was an improvement in pain and functional capacity between the initial evaluation and week 4, and the initial evaluation and week 8 in both groups (p < 0.05). In the quality of life evaluation, the criteria for pain, functional capacity, and physical aspects had significant improvement after 8 weeks (p < 0.05). There was no difference when comparing groups A and B (p > 0.05). CONCLUSION: Home-based exercise therapy, when performed in a period of eight weeks, using the booklet, was effective for improving level of pain, functional capacity, and quality of life in patients with non-specific chronic low back pain. The weekly supervision did not significantly influence the final outcome between the groups.

    Referral gynecological ambulatory clinic: principal diagnosis and distribution in health services

    Background: The association between gynecological diagnoses and their distribution in the health sectors provides benefits in the field of women's health promotion and in medical and interdisciplinary education, along with rationalization according to level of care complexity. Thus, the objective is analyze the clinical-demographic characteristics, main diagnoses in gynecological ambulatory care, and their distribution in health services. Method: This is a research project of retrospective audit study design with a chart review of data from 428 women treated at University Ambulatory Clinic of Women's Health, the facility in gynecology and training for Family and Community Medical Residents, Sao Paulo, Brazil, from 2012 to 2014. Clinical and demographic information, gynecological diagnoses (International Classification of Diseases), and distribution of health services (primary, secondary, and tertiary) were described. Results: The female patients present non-inflammatory disorders of the female genital tract (81.07%, n = 347) and diseases of the urinary system (22.66%, n = 97) among the gynecological diagnoses. The chances of having benign breast disease and non-inflammatory disorders of the female genital tract during the reproductive period corresponds to being 3.61 (CI 1.00-16.29) and 2.56 times (CI 1.58-4.16) higher, respectively, than during the non-reproductive period. The noninflammatory disorders of the female genital tract (93.33%, n = 28) are most related to the tertiary sector. The distribution in health services was the following: 71.30% (n = 305) in the primary sector, 21.70% (n = 93) in the secondary sector and 7% (n = 30) in the tertiary sector. Conclusion: The studied women presented non-inflammatory disorders of the female genital tract and diseases of the urinary system as determined by gynecological diagnoses. Low-assistance complexity followed in most cases.

    Knowledge and acceptance of HPV vaccine among adolescents, parents and health professionals: Construct development for collection and database composition

    Introduction: The human papillomavirus (HPV) is a prevalent viral infection in the sexually active population, which can be oncogenic and non-oncogenic. Educational efforts by health professionals, aimed at adolescents and their parents, help decision-making on human papillomavirus vaccination, benefiting the implantation process and vaccine coverage. Objective: To describe the data collection constructs about knowledge and acceptability of HPV vaccine among adolescents, parents and health professionals. Methods: Study of construct elaboration based on an empirical review of the literature with a qualitative focus on PubMed database, from 2007 to 2014, using the following keywords: Papillomaviridae AND Papillomavirus Vaccines AND Knowledge AND Community Health Services. A total of 31 questions were divided into six categories. In the internal validation, the final construct was applied in 390 subjects (adolescents, parents/guardians and health professionals) in the period of 2014. The proportion of assertive responses and respective 95% confidence interval (CI) were used to describe each question. Results: Three articles on the subject were found in the databases consulted that served as the basis for the elaboration of the questionnaire. There was a lower proportion of correct answers among adolescents about knowledge of HPV. Adolescents, parents, and carers showed a low proportion of correctness about the safety and efficacy of the vaccine. The three groups did not show any barriers to vaccine acceptability. Conclusion: The instrument was adequate to measure knowledge about HPV, its repercussions and its vaccine among adolescents, parents/guardians and health professionals, as well as measuring the acceptability of the human papillomavirus vaccine. © The authors (2018).

    Factors Associated with Adolescents' Choice to Use Long Acting Reversible Contraceptives: a Systematic Review

    Purpose of ReviewThis review aims to update the published literature to summarize our understanding about the associated factors with adolescents' willingness to use long-acting reversible contraceptives.Recent FindingsLong acting reversible contraception (LARCs), specifically intrauterine devices and implants, have been successful in preventing unintended pregnancy and repeat pregnancy among adolescents. Unfortunately, many misconceptions about eligibility, fears of the insertion procedure, and health effects prevent adolescents from choosing them.SummaryA total of 1316 articles were identified and only nine met the inclusion criteria. Included studies reported findings on 12,851 adolescents aged 12 to 19years old, revealing many correlates like the knowledge/information about these contraceptives, age, marital status, and cultural aspects. Few articles compared the correlates of choosing intrauterine devices versus implants. All articles presented studies performed in contexts where access barriers to contraceptives were removed, including the costs. In the future, it could prove useful to develop a study that could compare types of LARCs, as well as in an exclusive adolescent population in different countries. It would also be helpful to compare adolescents' use in low- and middle-income countries, and in different contexts with limited access to family planning services and contraceptives.
