    Lymphedema in Cancer Patients

    Metastasis to distant organs is the most important factor related to cancer survival. Tumor cells spread via blood and lymphatic vessels, systems responsible for transporting fluid and cells. Besides its structural role in cancer metastasis, the lymphatic system displays a critical functional interaction with developing tumors. In clinical settings, full evaluation of the lymphatic system involvement is crucial for treatment and prognosis in cancer patients. Comprehension of physiological and pathological processes involving the lymphatic system is a continuously growing field of basic and clinical research due to recent advances in molecular Lymphology. However, the most interesting biological aspects of lymphatic metastasis of cancer are beyond our objective in this chapter. Here we will focus on the consequences of cancer spread and therapeutic approaches and their impact on the lymphatic system. The knowledge of basic function, structure and distribution of the lymphatic system is essential to diagnosis and treatment in Oncology. In this chapter we will highlight relevant aspects of lymphatic anatomy as well as current topics in conservative and surgical treatment of lymphedema. © Springer Nature Switzerland AG 2022.

    Head and neck region is very rich in vascular structures and a site for every incident of malignant neoplasms with potential to nodal metastasis in the carotid sheath that, when operated, may require complex vascular reconstruction with potential morbid complications, especially in salvage surgery. But the region can also be affected by complex benignant neoplasms that may require concomitant endovascular and open approaches for proper treatment, besides the complexity of surrounding anatomical structures, and life-threatening bleeding events. © Springer Nature Switzerland AG 2022.

    Importance of concepts in abdominoplasty and liposuction in breast reconstructions

    In the USA, breast cancer is the second most common cancer in women after skin cancer. Approximately 12.3?% of women will be diagnosed with breast cancer at some point during their lifetime, based on 2010-2012 data. The number of new cases of breast cancer expected for Brazil in the year 2014 is 57,120, with an estimated risk of 56.09 cases per 100,000 women. One of the possibilities to reconstruct the breast is to use a myocutaneous transverse flap based on the rectus abdominis muscle (TRAM) and its variations. The good results obtained with this type of surgery are to achieve reconstructed breasts increasingly similar to normal, and it may also improve the abdominal contour of the patients. There are several concepts of aesthetic plastic surgery of the abdomen that can be transferred while reconstructing the breast with TRAM. Issues related to the incision, umbilicus, presence of bulging, sagging, and hernia must be considered, addressed, and evaluated. Many of the concepts of aesthetic abdominoplasty and liposuction are used in the abdominoplasty associated with the harvesting of the TRAM flap for breast reconstruction, in order to achieve better body contour for the patient. © Springer International Publishing Switzerland 2016.

    Lipoabdominoplasty combining a wide undermining of the upper abdomen with complete body liposuction

    Introduction: The evolution of techniques in abdominal surgery has lowered postoperative morbidity and complication rates and has continually encouraged surgeons to search for new innovations. This study describes an abdominoplasty technique that combines a wide undermining (2-3 cm lateral to the external border of the rectus muscles) with complete body liposuction, not preserving the Scarpa’s fascia. Methods: From January 2005 to May 2011, abdominoplasty associated with liposuction was performed on 203 patients with an average age of 45 years. Seventy-seven (37.93?%) patients were smokers. Results: The follow-up period ranged from 3 months to 5 years and 3 months (mean of 2 years). No patient showed suprapubic skin necrosis, infection, hematomas, or thromboembolism. Two patients showed seroma, eight hypertrophic scars and six dog ear scar revision. Conclusion: The technique presented in this study has a low complication rate and achieves a more extensive improvement of the body contouring. © Springer International Publishing Switzerland 2016.

    Aesthetic rehabilitation after bariatric surgery

    Sustained and lifelong weight loss for severely obese people is not anymore an impossible dream, and a Swedish article suggests, half jokingly, that this is the new fairy tale of the ugly duckling (Olbers, Lakartidningen 108:2570-2573, 2011). The example is appropriate because for health care professionals, resolution of obesity means reversal of diabetes, hyperlipidemia, and other cardiovascular risk factors. For the majority of the patients all these results are secondary. Priority is getting beautiful and achieving a rich and fulfilling emotional life. That is the reason why plastic interventions are the most frequent new operations after bariatric treatment, and as many as 85 % of this population desire subsequent body contouring maneuvers (Gusenoff et al., Plast Reconstr Surg 122:951-958, 2008; Highton et al. J Plast Reconstr Aesthet Surg 65:426-432, 2012). Fortunately this specialty has responded to the challenge, and both old and new techniques are being successfully adapted to the specificities of the patients who shed much weight and suffer from lax and redundant skinfolds and widespread lipodystrophy. Principal procedures such as abdominoplasty, mammaplasty, thigh lift, brachioplasty and others are described, emphasizing the experience of the Hospital das Clinicas Service during more than 15 years. © 2013 Springer-Verlag Berlin Heidelberg.     Transaxillary breast augmentation: The impact on axillary lymphatic drainage and sentinel lymph node evaluation

    Currently, alternative incisions for breast augmentation have been described, and among the techniques available, the axillary approach has gained status as an attractive technique due to the aesthetic results and the scar placement in a less visible position. However, the future impact of the procedure on the lymphatic drainage patterns and sentinel lymph node (SLN) detection remains controversial. To date little information is available regarding the feasibility of SLN identification in patients with previous transaxillary breast augmentation. The results of our studies and the lymphatic pattern observed suggest that the lymphatic breast drainage is more similar to a rich and extended network than to a few lymph trunks. According to some authors, based on oncological surgeries, even in the situation where this lymphatic channel is disrupted, due to the presence of a variable network of flexible lymphatics, the SLN has the potential to be mapped. However, additional long-term studies are necessary to investigate the accuracy of SLN biopsy in subgroups of breast cancer patients with previous breast implants. © 2015 Springer-Verlag Berlin Heidelberg.     Endothelial Function in Skin Microcirculation

    Cutaneous microcirculation is responsible for thermoregulatory exchange, excretion of several substances such as water and minerals and protection against external stressors such as heat and cold. Thus skin microcirculation plays a significant role in human body homeostasis as well as in several diseases that may affect the whole organism, including cancer, allergic reaction, dermatosis and dermatites. Two vascular plexuses compose the skin microvasculature: subdermal and dermic. Therefore this anatomical configuration allows endothelial function assessment by laser Doppler flowmetry. This chapter evaluates some skin conditions such as aging, collagen immune disease, and external noxious elements that can interfere with endothelial evaluation. © 2018 Elsevier Inc.     Sensibility of the ear after otoplasty

    Assessment of cutaneous tactile sensibility may be carried out by some neurosensory tests, based on the knowledge of skin innervation Dellon (Ann Plast Surg 51(1):6–9, 2003). The group A-beta nerve fibers are responsible for conducting touch stimuli including both those from slow- and fast-adaptation sensory receptors. The slow-adaptative fibers conduct pressure perception, constant touch, and texture, and their skin receptors are the Merkel and Ruffini cells. The fast-adaptative fibers provide motion and vibration perception, and their cutaneous receptors are the Meissner and Pacini cells. The authors report the results of their study of the sensibility of the ear preoperatively and postoperatively. © Springer-Verlag Berlin Heidelberg 2013.
