Abstract: Surgery for spinal deformity, even under the most experienced and skilled teams, brings risk to the neural structures. We discuss the main types of deformity surgeries and their critical steps, according to the surgeon, the anesthesiologist, and the neurophysiologist. The chapter covers our step-by-step protocol for intraoperative neurophysiology monitoring and describes in detail the steps of so-called transpedicular neurophysiological navigation. An algorithm for dealing with alarm signals is offered, based on the authors’ experience and literature review.
