    Adhesion molecules in primary oral mucosal melanoma: Study of integrins, claudins and immunoglobulins in a series of 35 cases

    Objective: Primary oral mucosal melanoma is a rare and aggressive tumor. Recent studies have reported a correlation between increased tumor invasion and metastatic phenotype with an altered expression of adhesion molecules profiles. The present study analyzed the expression of integrins, claudins and immunoglobulins in oral mucosal melanomas and correlated results with clinical features of patients. Method: Immunohistochemistry against these molecules was performed in thirty-five cases of primary oral mucosal melanomas organized into “tissue microarray ”and the results were related to clinical and histological features of patients. Results: Some subunits of integrins presented down-regulation and this was associated with vascular invasion. Increased expression of integrin beta3 and immunoglobulin CD166 (ALCAM) was statistically associated with cases with extensive vascular invasion (p <0,05). Lower expression of the immunoglobulin CD54 (ICAM) was associated with cases with extensive necrosis. Most cases with metastasis were negative for immunoglobulin CD66 (CEACAM). Several subunits of claudins showed down-regulation in the cases analyzed, and, although not statistically significant, it showed to be partially related with histological factors of poor prognosis. Conclusion: Altered patterns of adhesion molecules, mainly integrins and immunoglobulin seem to participate in the pathogenesis and outcome of oral mucosal melanomas.

    Pyostomatitis vegetans and its relation to inflammatory bowel disease, pyoderma gangrenosum, pyodermatitis vegetans, and pemphigus

    J Oral Pathol Med (2012) 41: 584588 Pyostomatitis vegetans (PSV) is an intraoral pustular eruption considered by most authors to represent the mucous analogous of cutaneous pyoderma gangrenosum and its vegetating presentations (pyodermatitis vegetans). A strong correlation of PSV with inflammatory bowel disease (IBD) is well documented. The histopathology of PSV lesions usually reveals acanthosis, and neutrophils and/or eosinophils infiltration with intraepithelial or subepithelial abscesses; acantholysis is present in some cases. We studied four patients with IBD that presented oral lesions suggestive of PSV. Two male and two female patients were included. The histopathology of oral lesions of two patients revealed findings typical for PSV. The other two patients showed findings typical for pemphigus vulgaris (PV), although the course of their symptoms paralleled that of the bowel disease. Our findings may suggest that pustular lesions in patients with IBD can be a presentation of both PSV and PV; adequate diagnosis is required because clinical presentation is very similar.

    MAP Kinase Pathways: Molecular Roads to Primary Acral Lentiginous Melanoma

    The etiology and pathogenesis of lentiginous acral melanomas are poorly understood. Recent studies have postulated that DNA repair mechanisms and cell growth pathways are involved in the development of melanoma, particularly changes in the MAPK pathways (RAS, BRAF, MEK 1/2, and ERK 1/2). The aim of this study is to assess the status of the MAP kinase pathways in the pathogenesis of acral melanomas. The authors examined the components of the RAS-RAF-MEK-ERK cascades by immunohistochemistry in a series of 16 primary acral melanomas by tissue microarray. The expression of MAP kinase cascade proteins changed in most cases. The authors observed that 57.14% of cases were BRAF positive and that 61.53%, 71.42%, and 71.42% of cases were positive for MEK2, ERK1, and ERK2, respectively; RAS was not expressed in 92.31%, and all cases were negative for MEK1. The absence of RAS and positivity for MEK2, ERK1, and ERK2 were most seen in invasive cases with high thickness. These aspects of the MAPK pathway require further examination in acral melanomas between different populations. Nevertheless, the results highlight significant alterations in the MAP kinase cascades that are related to histological indicators of prognosis in primary acral melanomas.

    Aphthosis is an inflammatory disease characterized by painful, recurrent oral ulcerations. The cause of aphthosis is unknown, and several studies indicate a role of the immune system in the physiopathogenesis of this disease. Oral lesions mainly appear on the non-keratinized mucosa as painful, regular, round, or oval ulcers covered by a gray pseudomembrane and surrounded by an erythematous halo. The diagnosis of aphthosis is mainly established on clinical grounds.     Nail Apparatus Melanoma: Experience of 10 Years in a Single Institution

    Background: Nail apparatus melanoma (NAM) is rare. The higher mortality reported in the literature is most likely due to advanced disease associated with delayed diagnosis and treatment. Objective: All patients diagnosed with NAM were followed at a single reference center during a period of 10 years, with emphasis on dermatological and histological findings. Clinical outcomes were included. Methods: A retrospective review of medical records, photographs, and histopathological examination. Results: 73.7% of the patients were female and their mean age was 47 years. Caucasian and African-American patients had a similar incidence (47.4%). There was only 1 Asian patient in this study (5.3%). All patients presented with nail plate pigmentation. The mean prediagnosis duration was 33 months. A total of 63.2% patients had in situ melanoma and an average Breslow index of 2.18 mm in invasive cases. Fifteen patients underwent conservative surgery and 3 underwent phalanx amputation. All patients survived, and 4 have had more than a 5-year follow-up. Conclusion: In situ lesions showed longitudinal bands or total melanonychia with no nail dystrophy. Invasive cases presented with dystrophic lesions (longitudinal splitting of the nail). Conservative surgery provides a good option without worsening the prognosis. All dermatologists should be aware of early signs of NAM, since prognosis depends on early treatment.
