    Queixas musculoesqueléticas em uma Unidade Básica de Saúde: implicações para o planejamento das ações em saúde e fisioterapia

    OBJECTIVE: The aim of this study was to analyze the prevalence musculoskeletal complaints in adults in a Health Center. METHODS: There were evaluated users seen in spontaneous reception in the period of 2010 March to 2011 May. Altogether there were studied 1.023 individuals. The characterization of the complaints was conducted through a questionnaire with socio demographic data and the reason for the demand for care. RESULTS: Data from this study showed that most users were women (71.2%), aged 31 to 60 years (50.0%), single (31.6%), retired (14.2%) and had complaints in various systems (77.1%). The musculoskeletal system was the most affected (14.4%), representing the second reason for seeking treatment (31.0%). Analyzing the odds ratios of occurrence of musculoskeletal complaints in relation to the variables studied, we found that people aged 40 to 59 years were 3.49 (95%CI 2.17-5.57) times more likely to occur in association with these pains than older and younger persons. There was no association between other systems and the variables studied. CONCLUSION: The high prevalence of musculoskeletal complaints requires a new look from health managers to meet these demands, thinking about the possibility of including the physiotherapist in primary health care unit to treat less complex pains.

    O acesso e o fazer da reabilitação na Atenção Primária à Saúde

    ABSTRACT For adequate rehabilitation service offer in Brazil, it is necessary to understand the availability of access to Rede Assistencial de Saúde (RAS) [Brazilian Health Care Network (HCN)] and develop practices to meet health needs. The objective was to estimate the distribution trend of rehabilitation human resources in HCN between 2007 and 2015, especially in Primary Health Care (PHC) and get to know the practice of Clínica Ampliada (CA) [Extended Clinical Care (ECC)], Projeto Terapêutico Singular (PTS) [Singular Therapeutic Project (STP)], and Apoio Matricial (AM) [Matrix Support (MS)] of speech therapists, physical therapist, and occupational therapists. We searched for professionals from the Health Care Network using Cadastro Nacional de Estabelecimentos de Saúde (CNES) [Brazilian National Register of Health Establishments (NRHE)], and the monthly trend was elaborated through Prais-Winsten linear regression models. Aiming at knowing the practices of ECC, STP, and MS, “Discourses of the Collective Subject” were made from interviews held with 12 professionals. Medium complexity services had higher concentration of professionals, except for the hospitals in São Paulo city, and PHC had the lower possibility of access. Though shy, healthcare increased for all three professionals, with emphasis on physical therapists in hospitals in São Paulo city, in a comparison between the state (0.73%) and city (0.95%). In PHC, the highest raises were for occupational therapists of São Paulo city and physical therapists in Brazil. For MS, ECC and STP, besides the difficulty of the working process itself, ideas such as “plurality of concepts,” “biopsychosocial perspective,” and “possibility of adaptation” of care were predominant. Despite the growing numbers, the availability of professionals is still low and uneven, concentrated in specialty, and emphasising the expansion of physical therapy in hospitals and occupational therapy in PHC.

    Impacto da dor musculoesquelética na incapacidade funcional

    ABSTRACT The objective is to estimate the prevalence of incapacity to perform tasks of daily living and to identify its association with pain and sociodemographic factors. This is a cross-sectional study, with a convenience sample, composed of individuals who actively sought healthcare in a small city with complaints of musculoskeletal pain and difficulties in carrying out activities of daily living. The sample comprised 766 individuals. Questionnaires were applied to assess the degree of difficulty in performing activities of daily living and pain (Nordic Musculoskeletal Complaints Questionnaire and Numerical Pain Scale). The prevalence of disability and pain was estimated, as well as five logistic regression models for disability were constructed considering sex, age, occupation and presence and characteristics of pain. The data found showed that the prevalence of some difficulty in performing activities of daily living was 87.6%, with a lot of difficulty 66.1%; musculoskeletal pain was 67.5%. The individuals were unable to perform 3.6 activities of daily living on average. Pain was the main association factor to explain it (OR 9.9; 95%CI 5.9-16.5), followed by age. Difficulty in performing activities of daily living was associated with pain in the lower limbs, with a frequency greater than four days a week, beginning more than five years ago and severe or unbearable intensity in crisis episodes. The prevalence of disability and pain was high. Musculoskeletal pain and age impacted functional disability. This study contributes to direct the construction of care actions that aim to minimize and prevent difficulties to perform daily tasks.

    Overview of physical therapy graduation courses in Brazil: current scenario

    Abstract Introduction: After the regulation of Physical Therapy (PT) in 1969, there were only six undergraduate courses in Brazil. In the 90s, higher education underwent major expansion in all professions and the same occurred to PT, with consequent increase in the number of professionals in the labor market and privatization of education. Objective: To describe the current situation of PT courses in Brazil offered by Higher Education Institutions (IES). Methods: The data for the region, academic organization, situation, period, school system, administrative category, vacancies, course hours and duration were obtained from the website of the Ministry of Education (MEC) and refer to the year of 2013. The descriptive analyzes of central tendency, dispersion and percentage were performed in Stata 9®. Results: From the total of 550 IES registered in the MEC, 281 (51%) were in the Southeast. Regarding the academic organization, 341 (62%) corresponded to universities and 483 (87.9%) of the IES were private. Of the courses, 521 (94.7%) are active, the predominant school system was the semiannual (91.5%) and 438 were part-time. The average vacancies authorized by the IES were 129 ± 102, with at least 44,900 vacancies available in the country. Of the courses, 75% had 4,000 hours of duration with the minimum of seven semesters and a maximum of fourteen. Conclusion: The data show a higher offer of PT courses in the private sector compared to the public. There was a progressive concentration of courses and vacancies in the Southeast, especially in São Paulo.

    Prevalence of Adolescent Idiopathic Scoliosis in the State of Sao Paulo, Brazil

    Study Design. A cross-sectional study. Objective. To estimate the prevalence of adolescent idiopathic scoliosis (AIS) in cities in the state of Sao Paulo, Brazil, as well as to identify demographic, clinical, and lifestyle factors associated with AIS. Summary of Background Data. AIS is a common three-dimensional spinal deformity. Epidemiological data about the condition in the southern hemisphere are scarce, and Brazil has no public health policies to implement school-based scoliosis screening programs. Methods. We assessed 2562 adolescents between 10 and 14 years of age. The screening procedure included measurement of the angle of trunk rotation using a scoliometer in the Adams forward bend test and the radiographic examination. Results. The overall prevalence of AIS was 1.5% (95% confidence interval [CI]: 1%-1.9%). The AIS prevalence was higher among the females than among the males-2.2% (95% CI: 1.4%-2.9%) and 0.5% (95% CI: 0.1%-0.9%), respectively. The following factors were associated with the development of AIS: being female (OR = 4.7, 95% CI: 1.8-12.2; P = 0.001) and being in the 13- to 14-year age group (OR = 2.2; 95% CI: 1.0-4.8; P = 0.035). Double curves and right laterality were more common (59.4% and 56.8%, respectively), although the curves were of low magnitude (75% of the curves having a Cobb angle <= 22 degrees), as was the progression factor (<= 1.2 in 75% of the cases). Conclusion. The prevalence of AIS in cities within the state of Sao Paulo was similar to that reported in the literature, was higher among females, and was higher during puberty (13-14 years of age). Because puberty occurs later for males than for females, the recommendation to screen both sexes at 10 to 14 years of age should be reconsidered.

    ABSTRACT To identify the profile of the physical therapist is important for a profession overview, which can help organs and institutions of higher education to train more qualified, fulfilled, valued, and recognized professionals for health care. The profile of the physical therapist has already been studied in other states, specific specialties or workplace; however we are not aware of the current profile of the physical therapist in the state of São Paulo (SP). The aim of this study was to describe the profile of the physical therapist of SP according to demographic, training, and professional performance aspects. All physical therapists registered in Crefito-3 with at least one year since graduation were invited to participate in the study, by means of answering an online questionnaire. In total, 2,323 physical therapists participated in the research. The sample was composed by: 80% women, 62% graduated between 2001 and 2010, 83% graduated from a private university, and 66.7% had attended a post-graduation specialization course. Most of them worked as physical therapists and had physiotherapy as their only source of income. The predominant workplace was home care and the monthly gross income was up to R$ 3,000.00 in 45% of cases. Professionals who were graduated for a longer time, those who work in universities and in their own clinics were better remunerated. Most professionals are satisfied with their profession, often attend update courses, present low adherence to unions and associations and are unaware of the current legislation in force. There is a predominance of female physical therapists in the state of São Paulo, young, who attended a post-graduation specialization course and who seek to be updated, but with low participation in the profession's institutions. Many are self-employed, professionals working in universities and in their own clinics are the best paid and the time of training and experience are relevant factors to achieve a better monthly income.

    Prevalence of musculoskeletal pain and impact on physical function and health care services in Belterra/PA

    Abstract Introduction: Musculoskeletal pain worth being investigated for being a multidimensional prevalent phenomenon, difficult to be understood which may have as main factors: tissue injury as well as emotional, social and environmental aspects. It is essential to know the specificity of the pain to define strategies for disease prevention and health promotion of the population. Objective: To estimate the prevalence of musculoskeletal pain, describing its features and functional implications in population from Belterra/Pará. Method: In a descriptive study, there were evaluated subjects who participated in activities of the project Bandeira Científica. Prevalence of pain was estimated with a confidence interval of 95%. Descriptive measures were used to characterize pain and consequences for functionality. Results: 453 subjects were interviewed, the mean age was 44.3 years old (SD = 18.0) and 69.6% were women. Prevalence of chronic pain was 62.5% and of back pain was 55.0%. Almost daily pain was felt by 67.9% and by 69.6% the intensity was strong or unbearable during crisis. Individuals with unbearable intensity and daily frequency of symptoms reported difficulty to perform heavy activities (91.5%) and to remain in the same position (82.9%). Conclusion: The prevalence of pain was high, of chronic feature, with high intensity and high frequency, long duration, most frequently in the vertebral column. The pain had interference in the performance of heavy activities and maintaining the same posture in people with greater intensity and frequency of pain.
