    Desenvolvimento histórico-epistemológico da epidemiologia e do conceito de risco

    Statistical reasoning in health practice was disseminated by epidemiology and is widely used in various health fields. This paper revisits the historical development of epidemiology and the concept of risk, responsible for the use of statistical chance as part of causal thinking in health sciences. The study covered the period from 1872 to 1965, and the documental base consisted of scientific articles related to the development of epidemiology as a field, particularly in the American Journal of Hygiene, as well as scientific books. Three phases were identified in the development of epidemiology: constitution, exposure, and risk. The article proceeds to discuss epistemological and social and health aspects required for the historical understanding of each phase. It concludes by stressing the relevance of critical reflection on epidemiological science and its relationship to health practice, especially in public health, in order to optimize its current use and foster its on-going creative reconstruction.

    Crise não epiléptica psicogênica: história e crítica de um conceito

    This discussion of the evolution of psychiatric knowledge concerning psychogenic non-epileptic seizures (PNES) sheds light on the epistemological assumptions underlying the concept and on its practical implications as well. PNES are defined as repeated seizures or attacks which can be mistaken for epilepsy because of the similar behavioral changes displayed, but which differ in that they are not the result of abnormal electrical activity in the brain and may be psychogenic in origin. The article investigates the historical development of the concept of PNES over the past forty years. The concepts of psychiatric comorbidity, abuse, and dissociation enter the discussion owing to their roles in the checkered development of the concept of PNES.

    A sexualidade de Adolescentes Vivendo com HIV: direitos e desafios para o cuidado

    Sexuality and reproductive healthcare represent relevant issues for comprehensive care of HIV-positive adolescents. However, public policies and health services give this issue insufficient attention. The scope of this article is to assess how HIV-positive young people and teenagers cope with their sexuality, dating and the urge to have children and start a family. In a qualitative study, in-depth interviews were staged with 21 HIV-positive (contracted by vertical, sexual or intravenous transmission) teenagers and 13 caregivers of children and youths living in Sao Paulo and Santos. The interviews revealed the different ways teenagers cope with their sexuality and with the anxiety of HIV disclosure in this context. Lack of information about HIV prevention, lack of support and skills to cope with their sexuality were revealed in the reports. Furthermore, stigma and discrimination were the most frequently reported difficulties. The main challenges to be faced in Brazil in regard to this issue are discussed, especially the need to consider HIV-positive youth as entitled to sexual rights. Recommendations are also made for incorporating the issue into a humanized and comprehensive care approach
