    Orientação teórica e os cenários de prática na formação de terapeutas ocupacionais na atenção primária à saúde: perspectivas de docentes

    Objective: This article aimed to describe and analyze the theoretical guidance and practical scenarios in the training of occupational therapists in Primary Health Care (PHC). Method: This is a qualitative research, which presents the perspectives of 17 teachers from nine occupational therapy courses in the state of Sao Paulo. Results: We categorized subjects associated to the study objective and present two categories: theoretical orientation - which includes the study of the referential Alma Ata (1978); Health Reform; critical understanding of the Unified Health System, the PHC and the profession specificity in this field involving understanding of the devices used by the teams occupational therapist and their working tools, social vulnerability understanding, everyday life and institutional context involving services and their management. The Practice Scenarios has been identified a plurality of services and drawings management where theoretical activities practices are developed, pointed out the difficulty of conducting training for this professional field. Moreover, the absence of workers on PHC hinders health care and also impairs the students training from different professional areas. Conclusion: The professional category must value since training, the learning of new care technologies compatible with their knowledge and occupational therapeutic resources towards the population's access to different practices for comprehensive health care, including the care of occupational therapy in PHC.

    Challenges on network care considering the perceptions of preceptors of a Pet-Network regarding people with disabilities and at-risk infants: access, comprehensiveness and communication

    Introduction: Health Care Networks (HCN) are seen as a possibility for articulating and coordinating actions, health services and is a priority in the health system. One strategy to assure their improvement is the Tutorial Education Program (PET-Redes) in the health area. Within this context, it is important to understand the challenges and strengths for the effectiveness of HCN to the network care. Objective: To identify and analyze the perceptions of professionals who are preceptors of PET-Networks (Stork/at-risk infants Network and Disabled People Network) about the configuration of the network care and the challenges of professional actions related to it. Method: Qualitative study conducted through two focus groups with professionals that are preceptors of the PET-Networks. Thematic analysis was used for data analysis. Results: Both networks presented the following categories related to the network care configuration: access, comprehensive health care and communication. For this care network configuration, some challenges mentioned include geographical and architectural barriers, lack of social support, lack of transportation, failure on care flow and discontinuities in the communicative process. To deal with these challenges, the following strategies were mentioned: implementation of electronic medical records and formulation of specific forums for debate and articulation of actions, among others. It is important to notice that these categories are intertwined and that more challenges than strategies to overcome them were pointed out by the participants. Conclusion: There are several challenges for effective HCN from different levels and the construction of strategies needs to be convened among all persons involved in the HCN.

    Referred speech-language and hearing complaints in the western region of Sao Paulo, Brazil

    OBJECTIVE: The aim of this study was to characterize the epidemiological profile of the population attending primary health care units in the western region of the city of Sao Paulo, Brazil, highlighting referred speech-language and hearing complaints. METHOD: This investigation was a cross-sectional observational study conducted in primary health care units. Household surveys were conducted and information was obtained from approximately 2602 individuals, including (but not limited to) data related to education, family income, health issues, access to public services and access to health services. The speech-language and hearing complaints were identified from specific questions. RESULTS: Our results revealed that the populations participating in the survey were heterogeneous in terms of their demographic and economic characteristics. The prevalence of referred speech-language and hearing complaints in this population was 10%, and only half the users of the public health system in the studied region who had complaints were monitored or received specific treatment. CONCLUSIONS: The results demonstrate the importance of using population surveys to identify speech-language and hearing complaints at the level of primary health care. Moreover, these findings highlight the need to reorganize the speech-language pathology and audiology service in the western region of Sao Paulo, as well as the need to improve the Family Health Strategy in areas that do not have a complete coverage, in order to expand and improve the territorial diagnostics and the speech-language pathology and audiology actions related to the prevention, identification, and rehabilitation of human communication disorders.

    The interface of occupational therapists practices with regards primary health care attributes

    Introduction: The specific dialogue of occupational therapy with Primary Health Care (APS) has weaknesses, which requires in-depth understanding. Objective: To identify and analyze the practices of occupational therapists in APS and its interface with the essential and derived attributes of this level of care. Method: Research of mixed methods (explanatory sequential) through three phases: Phase 1 - a collection of quantitative and qualitative data by an Internet Surveys with 105 occupational therapists from APS; Phase 2 - qualitative approach through the Grounded Theory, through interviews and observations of the practice with 8 professionals from the first phase; Phase 3 - Discussion and analyzing of results. Results: Phase 1 - 93.3% of the participants were women, mostly from state capitals and metropolitan regions, and from the Extended Family Health and Primary Care Center (NASF-AB). Regarding the orientation of their practices by APS attributes, the following results were identified: first contact attention (92.3%), longitudinality (84.7%), completeness (95.2%), care coordination (83.8%), family orientation (90.4%), community orientation (76.1%) and cultural competence (60.9%). Phase 2 - Eight NASF-AB occupational therapists from southeastern and northeastern capitals and metropolitan regions reported the work process in APS and their analysis indicated the following categories: practice characteristics and rationale, occupational therapy in clinical and pedagogical support, and challenges of practice. Conclusion: It was possible to verify that the occupational therapists' practices interface with the APS attributes, which indicates their relevance and insertion in this level of care.

    Uma experiência de uso do georreferenciamento e do mapeamento no processo de territorialização na Atenção Primária à Saúde

    ABSTRACT This article aims to report the experience of using tools that instrumentalize the process of territorialization in Primary Health Care. As methodological resources to guide the insertion and apprehension of problem situations, participant observation and field diary were used. The insertion with Family Health and Primary Care Extended Centers (Nasf-AB) provided the apprehension of the problems, and, with this, it was decided to develop mapping and georeferencing in order to collaborate with the process of territorialization of the unit, visibility of services and populations. The process of territorialization, which goes beyond bureaucratic incidents, has proved to be a challenge for professionals. It was possible to build tools with the Basic Health Unity teams in question, that could be north to other teams and health equipment.

    Avaliação da atenção primária em saúde por usuários com e sem deficiência

    ABSTRACT Purpose To evaluate the core (First Contact, Longitudinality, Comprehensiveness, and Coordination of Services) and derivative (Family Orientation, Community Orientation) attributes of primary health care (PHC) from the perspective of users with and without disabilities. Methods Observational, cross-sectional study using the Primary Care Assessment Tool (PCAT) with users with and without disabilities of five basic health units (BHU) in a municipality where 55% of the population depends on the Brazilian Unified Health System (SUS). Scores were assigned to the responses given to each of the PHC attributes evaluated. Results Study participants were 93 (67 physically disabled and 26 without disabilities) PHC users. No statistically significant differences were observed for any attribute on the comparison between the populations. For both groups, the attributes Degree of Affiliation, First Contact - Utilization, Longitudinality, and Coordination of Services - Information System received satisfactory (above cutoff) scores, whereas the attributes First Contact - Accessibility, Coordination of Services - Care Integration, Comprehensiveness, Family Orientation, and Community Orientation received unsatisfactory (below cutoff) scores. Users reported that the health teams are able to satisfactorily identify mobility issues, but there are failures in the recognition of problems of hearing, voice/speech and vision, and in the orientation of services available and services provided. Conclusion Users with and without disabilities evaluated the health care received similarly, indicating fragilities on the recognition of specific demands. Structural and work process changes should be conducted to ensure Accessibility, Comprehensiveness, and Family and Community Orientation, and thus increase the quality of PHC.

    Evaluation of at-risk infant care: comparison between models of primary health care

    OBJECTIVES: To analyze the health care network for at-risk infants in the western region of the city of sao Paulo, with the primary health care as coordinator, and to compare the presence and extension of attributes of primary health care in the services provided, according to the service management model (Family Health Strategy and traditional basic health units). METHODS: A survey was conducted with all at-risk infants born in the western region of Sao Paulo between 2013 and 2014. The children were then actively searched for a later application of the PCATool - child version. The total of 233 children were located in the territory; 113 guardians agreed to participate, and 81 composed the final sample. RESULTS: Regarding the results of PCATool for overall and essential scores, the units with Family Health Strategy were better evaluated by users, when compared with traditional basic health units, showing a statistically significant difference. However, these scores were low for both management models. Regarding attributes, the Family Health Strategy presented better performance compared with traditional basic health units for most of them, except for coordination of information systems. Of ten assessed attributes, seven reached values >= 6.6 for Family Health Strategy and two for the traditional basic health unit. CONCLUSIONS: Regardless of the type of management model, low overall and essential scores were found, indicating that guardians of at-risk infants rated some attributes as unsatisfactory, with emphasis on accessibility, integrality and family guidance. Such a performance may have negative consequences for the quality and integrality of these infants' health care.

    Situational diagnosis of people with disabilities, receiving occupational therapy service in a basic health unit

    This work is a situational diagnosis of adults with disabilities, identified by a basic health unit, in the city of Sao Paulo, SP. We sought to recognize their main health needs in order to contextualize, expand assistance and improve interdisciplinary action. The research was conducted in two phases. 1. through medical chart and Basic Assistance Information System review, for information of social-demographic as well as health aspects of 139 adults. 2. home visits and interviews with 21 adults with any type of limitation, aiming to detail the situational diagnosis. The results showed a higher number of male subjects, of working age, but excluded from the labor market, with low education and fewer opportunities of access to goods and services. The most sought care facilities were the Basic Health Unit and the hospital service, with emphasis on receiving clinical care. However, the interviewed identified needs in the rehabilitation treatment field. The study of living conditions, transport difficulties, access to culture and leisure activities, as well as perception and experiencing of violent situations, pointed to related factors that may facilitate some social vulnerability and impair access to social rights. The recognition of such needs is mandatory to plan and implement contextualized actions considering this population's critical life conditions in an effort to build a comprehensive and articulated care network.
