    Heart rate variability and depression

    Heart rate variability (HRV) is defined as the changes in the time intervals between consecutive heartbeats. These changes are expected to occur and reflect the autonomic nervous system (ANS) activity. Depression has been associated with reduced HRV particularly with high-frequency power HRV (HF-HRV), standard deviation of NN intervals (SDNN), and root mean square of successive NN interval differences (RMSSD) indices. The concept that low-frequency power HRV (LF-HRV) is an index of sympathetic tonus has been questioned and whether it is reduced or increased in depressed subjects is still controversial. The LF/HF ratio was found to be increased in two meta-analyses, however, its role as an index of sympathetic/parasympathetic imbalance has been questioned. Treatment of depression may be followed by amelioration of some HRV parameters, but most antidepressants decrease HRV. The reduction of HRV is traditionally considered as one of the mechanisms in the association of depression and cardiovascular disease (CVD). However, data support that HRV changes may precede depression, being a mark of decreased parasympathetic tonus, which would represent a vulnerability do depression. © 2021 Elsevier Inc.
