    Intervention of occupational therapists with frail elderly

    Introduction: Occupational therapists make up the teams that provide assistance to frail elderly people in different care contexts, including health, social assistance and culture. Objective: To identify the role of occupational therapists who work with frail elderly people. Method: Qualitative and exploratory study. Interviews were carried out with occupational therapists who work with frail elderly people in the city of Sao Paulo, SP, Brazil, using a semi-structured script of questions. ""Snowball sampling"" was used. The results were analyzed using thematic categorical content analysis. Results: Nine occupational therapists working in health, social care and culture were interviewed. The categories found were: I) actions developed by occupational therapists in assisting frail elderly people; II) teamwork and articulation of knowledge, contributions and challenges; III) articulation of networks and intersectoriality; IV) relevant aspects in comprehensive care for frail elderly people. The interviewees perform diversified actions, considering the cultural and social insertion of the elderly and public policies; shared with team professionals and with services from the sectorial and intersectorial network; included caregivers in the assistance provided to the frail elderly. Conclusion: Among the challenges faced by occupational therapists in working with frail elderly people, there is the incorporation of the premises of integrality, intersectoriality and teamwork. The different actions taken are supported by public policies for aging, and seek unique care based on biopsychosocial complexity and contextual aspects that influence aging. The results indicate the need to expand the insertion of occupational therapists in teams that offer care to frail in different care contexts.

    Experiência de terapeutas ocupacionais na atuação com idosos com comprometimento cognitivo leve

    Introduction: The Minor Neurocognitive Disorder, commonly known as Mild Cognitive Impairment (MCI), is one of the condition in which the primary clinic deficit is in the cognitive function and it brings repercussions on daily activities, especially to those who demands mostly cognitive abilities to its performance. The occupational therapist (OT) is a professional who can work with cognitive rehabilitation, directing their efforts to help patients perform occupational roles with a minimum interference of the cognitive limitations. Objective: To know the interventions performed by OT with elderly people with MCI. Method: A qualitative, exploratory and descriptive study with 10 OT active in Sao Paulo, in the areas of geriatrics and gerontology, with elderly with MCI. Participants were recruited using the technique snow-ball. Individual interviews were conducted with a semi-structured questionnaire, and the data were analyzed through thematic content analysis. Results: The OT reported performing individual and group calls to people with MCI, aiming to prevent cognitive decline, cognitive and functional improvement, maintenance of autonomy, improved quality of life and offering emotional support. Conclusion: It was found that the OT is a professional who has been increasingly recognized to work with the elderly with MCI, especially regarding to the adoption of features that improve or reduce difficulties in performing daily activities that involve more directly cognitive aspects.

    Promoting cognitive health: a virtual group intervention for community-living older adults

    ABSTRACT Group cognitive interventions can promote a sense of self-efficacy to older adults. Due to restrictive social distancing measures in the COVID-19 pandemic, face-to-face interventions that aimed to promote cognitive health needed to be adapted to a virtual offering. Objectives: This study aimed to analyze the effects of promoting cognitive health in a virtual group intervention for community-living older adults. Methods: This is a mixed, prospective, and analytical study. Before and after the intervention, the tests were applied: Brief Cognitive Screening Battery (BCSB) and the Subjective Memory Complaints Questionnaire (MAC-Q). Data were collected at semi-structured interviews related to the adoption of memory strategies. Statistical tests were conducted for initial and final intragroup comparison. The qualitative data were assessed using thematic analysis. Results: A total of 14 participants concluded the intervention. With respect to mnemonic strategies, the most relevant for the qualifier “Did not use it before and started to do so after the group” were association (n=10; 71.4%) and dual-task inhibition (n=9; 64.3%). According to the tests, the intervention improved incidental, immediate, and delayed recall, as well as the perception of memory for “Remembering the name of the person they just met,” “Remembering the telephone number you use at least once a week,” “Remembering where they put an object,” “Remembering news from a magazine article or television program,” and “In general, how would you describe your memory now compared to when you were 40 years old.” Conclusions: The synchronous virtual group intervention was shown to be feasible for the elderly in the community who participated in the study.

    Dificuldades para atividades básicas e instrumentais de vida diária, referidas por usuários de um centro de saúde escola do município de São Paulo

    Elderly who match certain demographic, social and economic criteria are especially vulnerable to illness, disability and death. The study was developed in order to both identify vulnerable elderly according to specific criteria and compare vulnerable and non-vulnerable elderly with their need for assistance with activities. This cross-sectional study had its data collected in the household of 190 elderly users of a School Health Center. It took place between 2006 and 2008, through the application of the CICAc Instrument: classification of the aged concerning self-care ability. One hundred and forty-five elderly (80%) were under at least one risk criterion; 99 (52.1%) reported one difficulty or more in basic activities, and 29 (29.3%) of them require assistance; 92 (48.4%) elderly mention difficulties in instrumental activities and 67 (72.8%) require assistance. Only 16 (8.4%) adopt self-care practices to minimize their difficulties and 38 (20%) adopt them to compensate just part of their difficulties. The elderly group that is under at least one risk criterion has a higher proportion of individuals who require assistance with basic and instrumental activities of daily living. Further analysis may verify the influence of each criterion on the functionality. Carrying out research on self-care practices with a larger number of individuals will allow a comparison between groups that make or do not make use of these practices and match risk criteria. Despite the limitations of the study, it gives rise to the planning of interventions for the elderly that have difficulties and need assistance with their activities.

    Programa de orientação com ênfase em práticas de autocuidado para motoristas idosos

    Abstract Objective: To present the results of an orientation program for elderly drivers which emphasized practices of self-care. The secondary objective was to describe the personal difficulties and environmental challenges identified by participants when driving a vehicle. Method: An intervention study of elderly drivers was performed. It included individual interviews, an orientation program and a focus group session. Results: A total of 13 elderly persons, aged between 62 and 82, participated in the study. Seven (54%) reported difficulties in driving, of whom five (71.4%) mentioned difficulties in interacting with the environment; three (42.9%) described emotional difficulties and three (42.9%) cited physical, sensory and/or cognitive difficulties. In the focus group session, the elderly persons reflected on factors addressed during the program that interfered with the act of driving, reporting that they had increased their self-care practices to cope with these. A majority of the participants identified the influence of attention deficits and the adverse effect of emotional issues when driving a vehicle. In terms of conditions relating to the social and physical environment, the elderly persons criticized the uncontrolled growth of traffic, mentioned their concerns about other drivers and pointed out deficiencies in the mechanisms of education, traffic control and punishment of traffic violations. They also discussed potential changes to encourage the mobility of pedestrians, a condition also experienced by the drivers. Conclusion: The elderly persons elucidated the importance of orientation in self-care practices, highlighting the importance of a dialogue between professionals and the elderly to discuss a wide range of practices related to factors that affect vehicle driving in order to maintain safety when driving for as long as possible.
