    Analysis of ocular cyclotorsion in lying position after peribulbar block and topical anesthesia

    Purpose: Evaluate the magnitude of cyclotorsion during cataract surgery in patients with indication for intraocular toric lenses comparing the results after peribulbar and after topical anesthesia. Methods: This prospective study comprised 112 eyes that underwent cataract surgery with implantation of toric intraocular lens by topical anesthesia or peribulbar block. We estimated how many degrees of cyclotorsion occurred after topical anesthesia and peribulbar block with the patient in supine position. A tag was performed in the position of 180 degrees of the right eye and zero degrees of the left eye, with the patient seated. Afterwards, it was requested a change to the supine position and then a new dial in 180 and zero degrees respectively from right and left eye were made. Results: The current study demonstrated that patients submitted to cataract surgery with implantation of toriclens under local anesthesia showed approximately 6.89 degrees of incyclotorsion (82 eyes) and 6.93 degrees of excyclotorsion (38 eyes) and a mean of cyclotorsion of 6.91 degrees. Patients undergoing peribulbar block showed 5.68 degrees of incyclotorsion (73 eyes) and 4.81 degrees of excyclotorsion (47 eyes) and a mean of cyclotorsion of 4.92 degrees. Conclusion: Through the study we can see that the movement of incyclotorsion in patients undergoing peribulbar anesthesia was lower when compared to topical anesthesia. This is relevant since the greater the incyclotorsion, the lower the predictability of the surgery and the lower the chance of obtaining excellent results in the final refractometric.

    Aplicação da mitomicina C na cirurgia refrativa

    ABSTRACT Over the years, mitomycin C has been used by refractive surgeons to prophylactically decrease haze after surface ablation procedures and therapeutically in the treatment of preexisting haze. Development of mitomycin C treatments has had a significant role in the revival of surface ablation techniques. We reviewed the literature regarding mechanism of action of mitomycin C, its role in modulating wound healing after refractive surgery, and its safety and efficacy as adjuvant therapy applied after primary photorefractive keratectomy surgery or after photorefractive keratectomy re-treatment after laser in situ keratomileusis and other corneal surgeries and disorders. The drug is a potent mitotic inhibitor that effectively blocks keratocyte activation, proliferation, and myofibroblast differentiation. Many studies have suggested that mitomycin C is safe and effective in doses used by anterior surface surgeons, although there continue to be concerns regarding long-term safety. After initial depletion of anterior keratocytes, keratocyte density seems to return to normal 6 to12 months after the use of mitomycin C when corneas are examined with the confocal microscope. Most clinical studies found no difference between preoperative and postoperative corneal endothelial cell densities when mitomycin C 0.02% was applied during refractive surgery,with exposure time of 2 minutes or less. After approximately 14 years of use, mitomycin C has been found to be effective when used for prevention and treatment of corneal haze.

    Cross-linking da cornea: protocolo padrão

    ABSTRACT The purpose of this review is to determine the indications and efficacy of the surgery that promotes new covalent bonds between the collagen fibers of the cornea, known as Cross-Linking (CXL), as well as to clarify its objectives. Keratoconus is a bilateral, asymmetric ectasic disease of the cornea that mainly courses with steepening and progressive thinning, and usually begins in the second decade of life. The primary use of CXL has been in discontinuing the progression of keratoconus. Although known corneal stromal remodeling occurs in these patients, the pathophysiology behind keratoconus is still unknown and appears to be multifactorial. By the literary evidence available to date, the CXL of the cornea is safely indicated in patients with progressing disease. We conclude that there is sufficient evidence to affirm that cornea CXL is effective in stabilizing corneal ectasic disease.

    Complicações primárias da interface do 'flap' de LASIK

    ABSTRACT Laser in situ Keratomileusis (LASIK) is one of the most accepted techniques in refractive surgery today. Its predictability, reproducibility, safety and low incidence of side effects justify the wide diffusion of this technique. This is a laser surgical procedure which purpose consists in the modification of corneal structure, allowing a shift of the focal point of the eye. Thus, it is possible to correct refractive errors such as myopia, astigmatism and small degrees of hyperopia. The LASIK technique creates a corneal flap interface with the use of a microkeratome or femtosecond laser. Then, excimer laser is applied on the stroma bed followed by repositioning of the flap, creating an interface between the anterior and posterior corneal lamella. Despite the numerous advantages there are patologies that originate from primary complications of the interface which can caused confusion during diagnosis because they have similarities in their clinical presentation. The aim of this work is to define, characterize and compare those entities with the intention of making the diagnosis easier so that there is appropriate conduct. As result, this study presents recommendations for the management of each kind of interface disease through investigation by medical history and physical examination, in order to allow differential diagnosis between the main primary entities of the LASIK procedure interface.

    Facoemulsificação versus extração extracapsular no sistema público de saúde: análise de custos para o hospital, para o governo e para a sociedade

    Purpose: To compare the costs of cataract surgery for a public hospital, the government and the society between the techniques of phacoemulsification (PHACO) and extracapsular cataract extraction (EECP). Methods: A review based on several clinical studies between 2002 and 2010 at the Ambulatorial Surgical Center of the University of Sao Paulo. Results: The estimated expenses associated with medical supplies used for surgery were R$ 468.92 and R$ 259.96 in the PHACO and EECP groups respectivelly. The hospital expenditures in the postoperative follow-up was approximately R$ 16.40 (42%) lower in the PHACO. Regarding the estimated spending for Social Security, the EECP group cost an average, US$ 44.58 per patient more than the PHACO group.
