    Association of adductor pollicis muscle thickness and handgrip strength with nutritional status in hospitalized individuals

    Background: malnutrition is common in hospitalized patients and early diagnosis can contribute to better clinical and nutritional outcomes. Adductor pollicis muscle thickness (APMT) and handgrip strength (HGS) have been used to identify reductions in strength and muscle mass, associated or not with conventional methods. Objective: we aimed to correlate APMT and HGS with conventional anthropometric variables in hospitalized patients, and assess their relationship with nutritional status as evaluated by the Patient-Generated Subjective Global Assessment (PG-SGA) method. Methods: a cross-sectional study was conducted in patients of both sexes admitted to a University Hospital in Brazil. APMT, HGS, and conventional measures were used for anthropometric assessment. PG-SGA was used for the assessment of nutritional status. Results: the sample included 73 patients (66.9 +/- 9.6 years). Most patients were admitted for surgery procedures (53.4 %) and had an adequate body mass index (BMI) (47.9 %), while according to PG-SGA most patients (67.1 %) had some degree of malnutrition (B and C). Right-hand (R) APMT was significantly correlated with corrected arm muscle area (cAMA), calf circumference (CC), and HGS. Left-hand (L) APMT was significantly correlated with cAMA, arm circumference (AC), CC, PG-SGA score, and HGS. Both HGS values (R/L) were significantly correlated with CC, PG-SGA score, and APMT. Conclusions: APMT and HGS were significantly correlated with the conventional anthropometric measure CC. In addition, the significant correlation observed between HGS, APMT, and PG-SGA highlights them as complementary assessments of nutritional status in clinical practice and for research purposes.

    Clinically significant changes in health-related quality of life in head and neck cancer patients following intensive nutritional care during radiotherapy

    Purpose: This study aimed to explore whether adherence to intensive nutritional care during radiotherapy would avoid a meaningful worsening in quality of life in head and neck cancer patients; and whether adherence was associated with better nutritional outcomes. Methods: Observational prospective study that assessed head and neck cancer patients treated with radiotherapy at a large oncology hospital, between August 2018 and April 2019. The main outcome was minimal clinically important difference in quality of life, assessed with EORTC QLQ-C30 and EORTC QLQ H&N35, between baseline and 12 weeks. To illustrate clinically significant changes in quality of life over timeby adherence, a heat map analysis was performed. We also evaluated nutritional outcomes. Results: Eighty patients were included, half of them (53.8%) were considered adherent. There were no significant difference in quality of life between groups at baseline, with the exception of swallowing (p = 0.029) and coughing (p < 0.01). After treatment, the heat map demonstrated that adherent patients had nonsignificant clinical change in function scales, while non-adherent patients had a clinically significant worsening in physical, cognitive and social function. The prevalence of malnutrition increased significantly only in non-adherent patients (p < 0.01). Conclusion: Adherence to intensive nutritional care may be able to avoid a meaningful worsening in quality of life and result in better nutritional outcomes in head and neck cancer patients. Our results may help to increase the awareness of the assessment of adherence and minimal clinically important difference in quality of life for research purposes and clinical practice.

    Appetite Assessment of Hospitalized Cancer Patients in Brazil - A Validation Study

    OBJECTIVES: Appetite loss, a common symptom in cancer patients, contributes to worsened nutritional status. A validated specific tool to assess appetite is clinically useful for diagnosing and identifying symptoms and signs that could be reversed with nutritional and pharmacological therapies. The aim of this study is to produce a Brazilian Portuguese version of the Hill and Blundell visual analog scale (VAS) for appetite and investigate its validity among hospitalized cancer patients. METHODS: The original English VAS version was translated into Brazilian Portuguese in full accordance with the guidelines in the literature and adapted to the Brazilian context by conducting interviews and meetings with an expert committee until the final version was reached. Afterwards, the version was validated in hospitalized cancer patients in a cross-sectional study at Sao Paulo Cancer Institute (ICESP), where the relationships between breakfast intake (rest-ingestion index) and VAS were compared. The Spearman test was used to verify the correlation between the rest-ingestion index and the VAS ratings. RESULTS: Sixty-four patients with a mean age of 56.1 (+/- 12.3) years answered the Portuguese VAS version, and their breakfast intake was evaluated. The mean rest-ingestion index was 18.8%. The correlations between the rest-ingestion index (food acceptance) and three questions of the Portuguese visual analog scale version were inverse and significant: first question (rho -0.3028 p=0.0046), second question (rho -0.2317 p=0.0319) and third question (rho -0.3049 p=0.0043). CONCLUSION: The ""Appetite Assessment Scale of Brazilian Oncology Patients"" is a valid instrument to assess appetite in hospitalized cancer patients in Brazil.

    Energy, macronutrient and antioxidant consumption of cancer patients in clinical treatment: a cross-sectional study

    Introduction: Among the proposed treatments for cancer, chemotherapy is the most common. Due to the symptoms and toxicity triggered by chemotherapy, patients may experience reduced food intake, which may exacerbate or further reduce the immune system's ability to respond to treatment and protein calorie malnutrition. Objective: To evaluate the consumption of energy, macronutrients and antioxidants of patients with cancer in clinical treatment and compare it with the nutritional recommendations. Methods: Cross-sectional study with non-probabilistic sample design. The food frequency questionnaire was used to evaluate weekly food consumption. The consumption of energy, macronutrients and antioxidant micronutrients was estimated and the intake was compared with the nutritional need, using a 95% confidence interval. The Brazilian Table of Food Composition was used to calculate the amount consumed of each food. For the statistical tests, a 5% significance level was adopted. Results: In this study, 135 cancer patients participated (64.4% were women). The diagnosis of breast cancer was the most prevalent (29.6%) and 45.5% were overweight. The consumption of energy and macronutrients, in both sexes, did not reach the recommended level, as well as the antioxidants vitamin E and selenium. Zinc consumption was also below that recommended for men. Fresh fruits (78.5%), vegetables (76.3%) and rice (76.3%) were the foods consumed daily by most patients. Discussion: The inadequate consumption of zinc, selenium, vitamin E, macronutrients and calories found, may indicate the need to reinforce the activities of food and nutrition education in this population, given the importance of energy balance and other nutrients for the maintenance and/or recovery of nutritional status. Conclusion: The dietary intake of evaluated cancer patients did not reach the recommended levels of energy, macronutrients and some antioxidant micronutrients. Inadequate intake is a cause for concern, as it is an important component to assist in treatment and recovery of patients.

    Offering Oncology Services in a municipality of Minas Gerais, Brazil

    Introduction: Cancer is a major public health problem worldwide that has an impact on global morbidity rates.This study aimed to determine the clinical, assistance and epidemiological offer of oncology services provided to people with cancer in a municipality of Minas Gerais, Brazil. Materials and Methods: A descriptive cross-sectional study was conducted with a geographical approach to health using data from oncological medical records in a municipality of Minas Gerais between 2007 and 2017. Epi Info,TerraView and QGIS were used for data analysis. Results: 1914 patients received assistance, who were predominantly white males over 60 years old holding complete elementary education and household occupation. The most frequent clinical care variables included clinical oncology, type I cancer staging, cancer stabilization, skin cancer, adenocarcinoma, surgery, chemotherapy and no reason for non-treatment. An important contribution was observed in records after 2009, as well as a decrease in non-analytic cases. 94.7% of geocoding was made with a heterogeneous spatial offer in urban areas and clusters in different neighborhoods. Conclusions: The clinical, assistance and epidemiological offer proposed for oncology services has actually been carried out. Temporal, spatial and frequency of care offers indicate a substantial increase in healthcare coverage for people with cancer who have received treatment under this scenario.

    Identifying Patient-Reported Outcome Measures (PROMs) for Routine Surveillance of Physical and Emotional Symptoms in Head and Neck Cancer Populations: A Systematic Review

    The aims of this review were to identify symptoms experienced by head and neck cancer (HNC) patients and their prevalence, as well as to compare symptom coverage identified in HNC specific patient-reported outcome measures (PROMs). Searches of Ovid Medline, Embase, PsychInfo, and CINAHL were conducted to identify studies. The search revealed 4569 unique articles and identified 115 eligible studies. The prevalence of reported symptoms was highly variable among included studies. Variability in sample size, timing of the assessments, and the use of different measures was noted across studies. Content mapping of commonly used PROMs showed variability and poor capture of prevalent symptoms, even though validation studies confirmed satisfactory reliability and validity. This suggests limitations of some of the tools in providing an accurate and comprehensive picture of the patient's symptoms and problems.

    Adherence to intensive nutrition care in head and neck cancer patients undergoing radiotherapy

    Purpose The aim of this study was to determine the prevalence and effect of adherence to intensive nutritional care on nutritional outcomes and survival in head and neck cancer patients undergoing radiotherapy. Methods Three-hundred and seventeen head and neck cancer patients referred to intensive nutrition support during radiotherapy were retrospectively analyzed. Patients who missed less than 25% of their appointments with the dietitian were considered adherent. Primary outcome was percentage weight loss during treatment. Secondary outcomes were overall survival and patients' capacity to accomplish their caloric and protein recommendations. Logistic regression was used to examine predictors of weight loss and Kaplan-Meier to estimate survival. Results Less than half of the patients (n = 145, 45.7%) were adherent. Statistically significant less weight loss in the adherent group (42.8% vs 55.8%; p = 0.02) was found, despite no difference in energy or protein intake. Logistic regression models after adjusting for other variables demonstrated that adherence resulted in 43% protection from significant weight loss (odds ratio 0.57, 95% CI 0.34-0.97). Overall survival was not different between groups. Conclusion Findings demonstrated that patients who were adherent to weekly contacts with the dietitian had less weight loss, but not better survival or nutritional intake. Additional investigation of factors that may act as barriers or enablers for adherence could help improve the outcomes in this population.

    Adherence to nutritional interventions in head and neck cancer patients: a systematic scoping review of the literature

    Background Dietary counselling provided by a dietitian, with or without oral nutritional supplements, can impact on nutritional and clinical outcomes in head and neck cancer (HNC) patients undergoing radiotherapy. However, little is known about the role of adherence to oral nutritional interventions in this population. This review aimed to map the literature for evidence of adherence to oral nutritional interventions in HNC patients undergoing radiotherapy and to identify gaps in knowledge in this field. Methods A scoping review methodology was used to identify studies, extract data, and collate and summarise results. We searched Medline, Embase, Cochrane Central and CINAHL, from the earliest available time up to 8 January 2020. Results In total, 2315 unique articles were identified, 163 studies were assessed in full and niner were included in the scoping review. The use of different measures to assess adherence and variability in the timing of the assessments was noted across studies. Despite identifying studies that have measured adherence to oral nutritional interventions, very few studies monitored its influence on clinical and nutritional outcomes in HNC patients or reported factors related to adherence. Conclusions A robust evidence base is lacking for adherence to oral nutritional intervention in HNC patients. Overall, further studies evaluating the impact of oral nutritional interventions in HNC patients undergoing radiotherapy should measure adherence to the intervention. Early recognition of non-adherence and the contributing factors could ensure intensification of nutritional support and better health outcomes.

    Assessment of the fat mass index in women recently diagnosed with gynecological tumors

    Objectives: to evaluate the nutritional status and body composition of women with gynecological tumors and evaluate the fat mass index (FMI) as a complementary indicator for addressing the nutrition status. Methods: a cross-sectional study with women recently diagnosed with gynecological tumors. Nutritional status was assessed using conventional anthropometry and the Patient-Generated Subjective Global Assessment. For body composition, bioelectrical impedance was used. Results: a total of 158 women participated, most of them with excess weight and high body fat. The FMI showed a positive and significant correlation with body mass index, arm circumference, tricipital skinfold, and arm muscle circumference. Conclusion: women recently diagnosed with gynecological tumors had excess weight and high body fat. The FMI may be a potentially useful indicator to complement the assessment of nutritional status and help the multidisciplinary team to perform early clinical and nutritional interventions.
