    EFFECT OF PROGRESSION OF ADOLESCENT IDIOPATHIC SCOLIOSIS ON GAIT PARAMETERS

    ABSTRACT Objectives: To evaluate the influence of the degree of severity of adolescent idiopathic scoliosis (AIS) on the distribution of plantar load on the feet during gait. Material and Methods: 40 patients with AIS were evaluated and divided into severity groups: 13 with mild AIS; 13 with moderate AIS; and 14 with severe AIS. Cobb angles (degrees) were evaluated by radiography. Gait was assessed using the pressure platform at a frequency of 100 Hz. The adolescents walked on a 20-meter track, with their feet resting on the platform, totaling an average of 12 steps. The following were evaluated: contact area, peak pressure, and maximum force on the 4 regions of the feet: hindfoot (medial and lateral), midfoot, and forefoot. Results: Adolescents with moderate and severe AIS showed an increase in peak pressure and maximum force on the medial (p=0.014; p=0.045, respectively) and lateral (p=0.035; p=0.039, respectively) hindfoot and a reduction on the midfoot (p=0.024) when compared to mild AIS. The contact area showed no differences between groups. Conclusion: The moderate and severe degree of AIS severity promoted increased plantar load on the medial and lateral hindfoot (heel) during gait compared to adolescents with a mild degree of scoliotic curvature. In this way, gait training in the milder stages of disease severity can minimize the overload and the increase in force vectors on the spine, preventing the progression of severe scoliotic curvature.     EFFECT OF SURGICAL TREATMENT OF SCOLIOTIC CURVATURE ON THE SPINE AND UPPER LIMBS

    ABSTRACT Objectives: Evaluate the surgical treatment of scoliotic curvature effects and sagittal radiographic parameters of the spine and upper limbs after surgical treatment with direct vertebral rotation (DVR) associated with type 1 osteotomy in adolescents with idiopathic scoliosis (AIS). Material and Methods: 41 patients with AIS were evaluated and compared before and after surgery. Scoliosis was confirmed by a radiographic of the spine. The radiographic parameters evaluated were: Cobb angles (proximal and distal thoracic), segmental kyphosis (T5-T12), total kyphosis (T1-T12), cervical lordosis (C2-C7), distance from the center of gravity of the skull to C7 (Cervical VAS), measurement of T1 slope (T1), neck slope, IT slope (AP), angle of the upper chest opening and plumb line C7-S1 (SVA C7-S1). Statistical analysis compared the pre- and post-surgical effects of sagittal cervical and shoulder parameters. Results: After the surgical procedure, significant reductions were observed for the following parameters of spine measurement: proximal and distal thoracic Cobb angle, with a correction of 68% of the main thoracic curve. The measurements of the inclination of the T1 profile and the inclination of the neck also improve after surgery. Regarding the shoulders, there was a reduction in T1 AP slope and intercoracoid angle after surgery. The other radiographic parameters did not show significant differences. Conclusion: Surgical treatment with direct vertebral rotation (DVR) associated with type 1 osteotomy promoted better sagittal radiographic parameters of the thoracic Cobb angles, T1 inclination of the cervical spine, neck inclination, and better shoulder symmetry.     Exercise behavior, practice, injury, and symptoms of respiratory tract infection of 502 Brazilian adults during lockdown oscillations in two years (2021-2022) of the COVID-19 pandemic

    BackgroundIn the period between 2020 and 2023, during the COVID-19 (coronavirus disease 2019) pandemic, many countries released their restriction measures so that individuals were able to begin practicing physical exercises and outdoor sports again. The purpose of the current study was to evaluate the physical exercise behavior, symptoms of respiratory tract infection, and training practice, as well as aspects of pain and injuries in the lower limbs of adults during periods of lockdown oscillations in the two years of the COVID-19 pandemic in Brazil.MethodsCross-sectional study. Participants: A total of 502 adults were evaluated during two consecutive years of the COVID-19 pandemic, corresponding to the years 2021 and 2022. A virtual questionnaire was applied using the Google Forms platform through a link, or a Quick Response Code available in social media environments. The variables collected were: anthropometric characteristics, presence of comorbidities, clinical history for the diagnosis of COVID-19, and behavior related to physical exercise practices, divided into five topics: (1) physical exercise habits; (2) symptoms and health care utilization; (3) habit of practicing physical exercise in relation to the prevention of COVID-19; (4) preventive measures for COVID-19; and (5) feelings and reasons for practicing exercises.ResultsA total 79.0% of the participants returned to the practice of physical exercise after a period of social isolation due to COVID-19, with running (30.0%) and muscle strength training (50.0%) being the most prevalent modalities, in which 62.0% of practitioners carried out the activity individually, without any professional or technical monitoring. With regard to physical preparation, 61.0% reported performing pre-training stretching, 64.0% associated with muscular resistance training. Of these, 89% did not report current injuries or pain symptoms when returning to exercise (69.0%). Total of 60.5% reported experiencing respiratory tract symptoms of COVID-19 and seeking a consultation with a doctor, and 61.0% performed diagnostic test, with RT-PCR (Real time-polymerase chain reaction) being the most common test. Of those tested, 55.0% were positive for COVID-19, without the need for hospitalization (95.0%). The most commonly used measures for the prevention of COVID-19 were the fabric or surgical mask. The predominant feeling in the pandemic was anxiety (50.5%) and the reasons for practicing sports were: physical conditioning (30.9%), a feeling of pleasure (21.3%), and weight loss (20.3%).ConclusionAfter two years of the COVID-19 pandemic (2021-2022), with periods of lockdown, there were low reports of injuries and pain symptoms after exercising on the return to physical exercise practices of running and strength training. However, the restrictions negatively affected the exercise behavior due to respiratory tract symptoms of COVID-19 and a reduction in training intensity, performed without any professional or technical supervision. The participants reported the use of a fabric or surgical mask for the prevention of COVID-19, and an increased feeling of anxiety. The reasons given for practicing physical exercise were physical conditioning, a feeling of pleasure, and weight loss.

    y Relationships between static foot alignment and dynamic plantar loads in runners with acute and chronic stages of plantar fasciitis: a cross-sectional study

    Background: The risk factors for the development of plantar fasciitis (PF) have been associated with the medial longitudinal arch (MLA), rearfoot alignment and calcaneal overload. However, the relationships between the biomechanical variables have yet to be determined. Objective: The goal of this study was to investigate the relationships between the MLA, rearfoot alignment, and dynamic plantar loads in runners with unilateral PF in acute and chronic phases. Method: Cross-sectional study which thirty-five runners with unilateral PF were evaluated: 20 in the acute phase (with pain) and 15 with previous chronic PF (without pain). The MLA index and rearfoot alignment were calculated using digital images. The contact area, maximum force, peak pressure, and force-time integral over three plantar areas were acquired with Pedar X insoles while running at 12 km/h, and the loading rates were calculated from the vertical forces. Results: The multiple regression analyses indicated that both the force-time integral (R-2=0.15 for acute phase PF; R-2=0.17 for chronic PF) and maximum force (R-2=0.35 for chronic PF) over the forefoot were predicted by an elevated MLA index. The rearfoot valgus alignment predicted the maximum force over the rearfoot in both PF groups: acute (R-2=0.18) and chronic (R-2=0.45). The rearfoot valgus alignment also predicted higher loading rates in the PF groups: acute (R-2=0.19) and chronic (R-2=0.40). Conclusion: The MLA index and the rearfoot alignment were good predictors of plantar loads over the forefoot and rearfoot areas in runners with PF. However, rearfoot valgus was demonstrated to be an important clinical measure, since it was able to predict the maximum force and both loading rates over the rearfoot.

    The effect of direct vertebral rotation on the spine parameters (coronal and sagittal) in adolescent idiopathic scoliosis

    BACKGROUND: Idiopathic scoliosis is accompanied by postural alterations, instability of gait, and functional disabilities. The objective was to verify radiographic parameters (coronal and sagittal) of adolescents with idiopathic scoliosis (AIS) pre- and post-surgery with direct vertebral rotation (DVR), associated with type 1 osteotomies in all segments (except the most proximal) and type 2 in the periapical vertebrae of the curves. METHODS: A prospective study design was employed in which 41 AIS were evaluated and compared pre- and post-surgery. Scoliosis was confirmed by a spine X-ray exam (Cobb angle). Eight radiographic parameters were measured: Cobb angles (thoracic proximal and distal), segmental kyphosis, total kyphosis, lumbar lordosis, pelvic incidence, sacral slope, and pelvic tilt. RESULTS: The Cobb angle averaged 51.3 degrees +/- 14.9 degrees. Post-surgery, there were significant reductions for the following spine measurement parameters: Cobb angle thoracic proximal (p = 0.003); Cobb angle thoracic distal (p = 0.001); Cobb angle lumbar (p = 0.001); kyphosis (T5-T12, p = 0.012); and kyphosis (T1-T12, p = 0.002). These reductions showed the effectiveness of surgical correction to reduce Cobb angles and improve thoracic kyphosis. The values obtained for lumbar lordosis, pelvic incidence, sacral slope, and pelvic tilt were not significantly different pre- and post-surgery. CONCLUSION: The surgical technique of DVR in AIS proved to be effective in the coronal and sagittal parameters directed at Cobb angles and thoracic kyphosis in order to favor the rehabilitation process.

    POSTURAL ASSESSMENT OF LUMBAR LORDOSIS AND PELVIC ALIGNMENT ANGLES IN ADOLESCENT USERS AND NONUSERS OF HIGH-HEELED SHOES

    Objective: The aims of this study were (1) to analyze the influence of wearing high-heeled shoes on lumbar lordosis and pelvic inclinations among adolescents aged between 13 and 20 years were users and nonusers of high-heeled shoes and (2) to correlate these postural angles with age. Methods: Fifty adolescents from the nonuser group (NUG) of high-heeled shoes and 50 from the user group (UG) were evaluated. Postural assessments were obtained by photogrammetry under 2 conditions-barefoot and with high-heeled shoes-and analyzed using the evaluation postural software. The measured angles included lumbar lordosis and the horizontal alignment of the pelvis. Descriptive analyses were carried out, with a significance level of 5%. Results: With high-heeled use, the NUG demonstrated rectification of the lumbar spine and pelvic retroversion, whereas the UG demonstrated hyperlordosis and pelvic anteversion. When barefoot, smaller lumbar lordosis angles for both groups were observed. However, the pelvic angles were lower for the UG group and higher for the NUG. The studied angles showed high reliabilities. Age was correlated with lumbar lordosis angles for the NUG in the barefoot condition and with pelvic alignments in both conditions for the UG. Conclusion: For the subjects in this study, the use of high-heeled shoes is correlated with increased lumbar lordosis and pelvic anteversions. Lumbar lordosis angles are correlated with age for the NUG when barefoot. (J Manipulative Physiol Ther 2011;34:614-621)

    CORRELATION BETWEEN CYPHOSE AND LORDOSE WITH THE FOOT SUPPORT OF ADOLESCENTS WITH IDIOPATHIC SCOLIOSIS

    ABSTRACT Objective: To verify the correlation between the thoracic and lumbar Cobb angle and the type of foot, and the parameters of plantar support during gait in adolescents with idiopathic scoliosis. Material and Methods: Sixty adolescents with idiopathic scoliosis (AIS) were divided into three groups: normal foot (n=20), cavus foot (n=20), and flat foot (n=20). The Cobb angles of thoracic kyphosis and lumbar lordosis were evaluated by radiographic examination. The plantar arch was recorded the podoscope and calculated by the ratio between the midfoot and the total foot area. The adolescents performed the march on a 20-meter track, with their feet resting on the pressure platform, totaling an average of 12 steps of the foot (right and left). The variables evaluated were: contact area, peak pressure, and maximum force on the four regions of the feet: hindfoot (medial and lateral), midfoot, and forefoot. Results: There was a positive correlation between the Cobb angle of lumbar lordosis and the arch plantar cavus (r=0.40; p=0.048) and flat (0.25; p=0.004), with no significant correlations for the Cobb angle thoracic (p>0.005). The pressure peak strongly correlated with the cavus plantar arch (r=0.92, p=0.001) in the lateral hindfoot and forefoot region, while the flat foot with the midfoot region. Conclusion: The Cobb lumbar lordosis angle positively correlates with the plantar arch height and the plantar support pattern during gait in adolescents with idiopathic scoliosis.
