    ACE2/angiotensin-(1–7)/mas receptor axis in the central nervous system

    The renin–angiotensin system (RAS) is classically conceived as a circulating hormonal system involved in cardiovascular and renal homeostasis. The discovery that RAS components are locally expressed in the brain tissue pointed out to a role for this system in the pathophysiology of neuropsychiatric diseases, including mood disorders and neurodegenerative and cerebrovascular illnesses. The RAS counterregulatory axis composed by the angiotensin-converting enzyme 2 (ACE2), angiotensin-(1–7) (Ang-(1–7)), and Mas receptor mediates, among others, antiinflammatory, antioxidant, and antiapoptotic processes, frequently opposing the classical RAS arm (ACE/Ang II/AT1 receptor) actions. Accumulating evidence has supported protective roles of the ACE2/Ang-(1–7)/Mas receptor axis in the brain. Herein, we will discuss emerging evidence regarding the role of RAS, mainly focusing in the ACE2-Ang-(1–7)-Mas receptor arm, in brain physiology and pathophysiology. We will also report current experimental and clinical evidence in relation to ACE2 stimulation and Mas receptor agonists as potential therapeutic targets for neuropsychiatric diseases. © 2023 Elsevier Inc.     University educated man with childish behavior

    A 39-year-old man, a human resource executive, was seen at the outpatient clinic of the Behavioral and Cognitive Neurology Unit of the Hospital das Clínicas, a public university hospital in São Paulo, Brazil. His wife reported that about 4 years before, she noticed that he was manifesting mild behavioral and personality changes characterized by irritability, a tendency for social withdrawal, together with a more rancorous and even mean personality trait. Progressively, he gave up drinking alcohol, which he used to consume in moderate amounts, and libido became extremely reduced. Then he started to complain of difficulties in his professional activities and to manifest obsessive-compulsive behaviors such as to demand for perfectly ironed clothes or to soap his body several times during bathing. He had an acute and rapidly reversible episode of paralysis of his legs and right arm. After a few months, he had another similar episode involving only the right side of the body, including face. He was seen by physicians and after several tests the diagnosis of probable viral meningitis was undertaken. Test for HIV-1 infection was reported to be negative. He was dismissed from his job because he could not handle it any more. Ginkgo biloba and HydergineR were prescribed. His behavior deteriorated to a more childish one with frequent cries when his demands were not immediately fulfilled. Although he had had the old habit of speaking aloud about his problems, this trait became very much increased and associated with intense gesticulations. More recently, his wife also noticed a decrease of his memory. The patient complained of lack of concentration and difficulties in organizing his activities.     Brainstem Circuitry and Emotions

    The human brainstem forms the central axis of the brain and serves as an integrative hub for major critical processes for the individual's survival and homeostasis. Extending within the brainstem's tegmentum and basal forebrain, the reticular formation constitutes a subcortical network of diffusely arranged nuclei projecting throughout the entire central nervous system. These include the noradrenergic locus ceruleus in the pons; the serotonergic dorsal raphe nucleus and dopaminergic substantia nigra in the midbrain; and the cholinergic nucleus basalis of Meynert in the basal forebrain. Here we review the neuroanatomical and functional properties of the reticular formation and provide evidence demonstrating its crucial role in mediating neuropsychiatric functions including sleep homeostasis and mood disorders. Overall, a better understanding of the reticular formation properties and vulnerability in brain disorders will provide the necessary foundation for the development of therapies targeting these systems. © 2016 Elsevier Inc.
