Albeit a traditional ablative procedure, Gamma Knife radiosurgery continues to be indicated for some severe and otherwise untreatable psychiatric disorders, especially obsessive-compulsive disorder. The majority of studies published so far have used lesions at the anterior limb of the internal capsule and consisted of case series. However, a placebo-controlled, double-blind randomized trial was recently published. Symptom improvements have been described in 25-80% of patients, suggesting that it is probably as efficacious as deep brain stimulation. Refinements of the radiosurgical techniques have allowed to lesion smaller volumes of white matter tissue, and thus a decreased incidence of severe adverse events has been reported. Nevertheless, aberrant radionecrotic reactions, such as brain cysts, are sporadically observed in a long-term follow-up. © 2016 John Wiley & Sons, Ltd. All rights reserved.
